Over the course of the study, here are the tools that you will need and use frequently. We made it easy for you to keep track of them by listing each one:
STUDY DASHBOARD
What is the Study Dashboard? Your Study Dashboard allows you to track some of your completed, current, and upcoming study contributions. You can also see how much compensation you have earned by visiting the Study Dashboard. You can access your Study Dashboard by clicking HERE.
Where can I find a copy of my signed informed consent document, CA Bill of Rights and HIPAA Authorization? You can find a copy of your signed informed consent and HIPAA Authorization in your Study Dashboard. Click HERE to login and select:
- Settings
- Study Documents
- Download Informed Consent Document
Can I withdraw from the study? Once you are enrolled in the iLUMENate Study, you can withdraw from the study at any time prior to completing the study. If you withdraw, Study Data collection will end as described in the informed consent form.
To withdraw from the study, go to the Settings section of your Study Dashboard and click the “Withdraw from Study” button.
You can also email the support team at study+ilumenate@evidation.com to request that they withdraw you.
Please note that by withdrawing:
- You will no longer be able to participate in the study.
- You will not be able to rejoin the study.
- You will only be compensated for your completed study contributions. Your compensation will be sent to you within four weeks from the date you withdraw.
